Confidentiality Policy

Personal health information may be collected and used in order to provide speech and language services. Measures are taken to keep your health information confidential.

Verbal and/or written consent will be obtained before disclosing information to anyone (i.e. other health care providers).
Payment Policy

Payment is due immediately after each appointment. Payments may be made by cash, etransfer to annemarieweeda@me.com or cheque payable to Annemarie Weeda. A $20 fee will be applied for NSF cheques.
